Resumen:

Introducción: El cáncer de páncreas exocrino es una neoplasia agresiva que representa el 85 % de los tumores pancreáticos, y solo el 15-20 % es resecable al diagnóstico. Su tratamiento se basa en cirugía y quimioterapia, a menudo combinado con radioterapia.
Una alternativa es el tratamiento intratumoral con micropartículas de fósforo-32 (OncoSil™), administrado por ecoendoscopia, que permite la radiación localizada con pocos efectos en tejidos cercanos. Busca reducir el tumor y mejorar el control en casos irresecables.
Material y Método: Varón de 68 años con HTA y diabetes tipo 2, diagnosticado de adenocarcinoma ductal de páncreas localmente avanzado que rodea la arteria hepática común >180º, compresión de la vena mesentérica superior y estenosis de la segunda porción duodenal. Recibió cirugía paliativa, quimioterapia (FOLFIRINOX) y OncoSil con buena respuesta, y posteriormente fue operado mediante duodenopancreatectomía con márgenes negativos.
Discusión: La combinación terapéutica es segura y puede volver tumores resecables inicialmente irresecables. La planificación quirúrgica se beneficia de la reconstrucción 3D para evaluar la resecabilidad y estructuras vasculares.
Conclusión: El cáncer de páncreas es agresivo y suele ser irresecable al diagnóstico; la única opción curativa es la cirugía. La combinación de FOLFIRINOX y OncoSil puede reducir el tumor y permitir la resección quirúrgica R0.

Abstract:

Introduction: Exocrine pancreatic cancer is an aggressive neoplasm that accounts for 85% of pancreatic tumors, and only 15-20% are resectable at diagnosis. Treatment is based on surgery and chemotherapy, often combined with radiotherapy.
An alternative is intratumoral treatment with phosphorus-32 microparticles (OncoSil™), administered by endoscopic ultrasound, which allows for localized radiation with minimal effects on nearby tissues. It aims to reduce the tumor and improve control in unresectable cases.
Material and Methods: A 68-year-old male with hypertension and type 2 diabetes was diagnosed with locally advanced pancreatic ductal adenocarcinoma surrounding the common hepatic artery >180º, compressing the superior mesenteric vein, and causing stenosis of the second portion of the duodenum. He received palliative surgery, chemotherapy (FOLFIRINOX), and OncoSil with a good response, and subsequently underwent a pancreaticoduodenectomy with negative margins.
Discussion: The therapeutic combination is safe and can render initially unresectable tumors resectable. Surgical planning benefits from 3D reconstruction to assess resectability and vascular structures.
Conclusion: Pancreatic cancer is aggressive and is usually unresectable at diagnosis; the only curative option is surgery. The combination of FOLFIRINOX and OncoSil can shrink the tumor and allow for R0 surgical resection.
