Step-by-Step Surgical Technique for Minimally Invasive McKeown Esophagectomy

Resumen:

La esofagectomía de McKeown es un procedimiento quirúrgico para el tratamiento del cáncer de esófago, especialmente los localizados en sus tercios superior y medio. Su característica distintiva es la realización de una anastomosis esofagogástrica a nivel cervical, tras la disección de los compartimentos torácico, abdominal y cervical. El procedimiento se desarrolla en tres fases. En la primera fase de toracoscopia derecha en decúbito prono, se moviliza el esófago torácico incluyendo el mesoesófago y se realiza la linfadenectomía mediastínica incluyendo los grupos periesofágicos, intertraqueobronquiales y, en caso de indicación, los ganglios de las cadenas recurrenciales. En la fase de laparoscopia se moviliza el estómago preservando la arteria gastroepiploica derecha y se realiza la linfadenectomía del tronco celiaco y de las arterias hepática común y esplénica proximal. Se completa esta fase con una mini-laparotomía para extracción de la pieza quirúrgica y confección de la gastroplastia tubular. Finalmente, en la fase de cervicotomía izquierda se diseca el esófago cervical y se ascienden la pieza quirúrgica y la plastia gástrica y se realiza la anastomosis cervical. Este abordaje está especialmente indicado en el carcinoma escamoso de esófago torácico superior y medio, así como en casos que requieren linfadenectomía extendida por sospecha de afectación ganglionar supraclavicular o cervical. A pesar de ser una técnica estandarizada con una mortalidad baja en centros de alto volumen (~1%), la morbilidad postoperatoria alcanza el 50%. En comparación con la técnica de Ivor-Lewis, presenta mayor incidencia de fuga anastomótica, aunque de menor gravedad clínica, y mayor tasa de estenosis anastomótica y de lesión del nervio recurrente laríngeo.

Abstract:

McKeown esophagectomy is an established surgical procedure for the treatment of resectable esophageal a cancer. Its defining characteristic is the creation of a cervical esophagogastric anastomosis following the dissection of the thoracic, abdominal, and cervical compartments. The procedure is performed in three distinct phases:
o Thoracoscopic Phase: With the patient in the prone position, the thoracic esophagus is mobilized. A systematic mediastinal lymphadenectomy is performed, encompassing the periesophageal and intertracheobronchial nodes, as well as the recurrent laryngeal nerve chains when indicated.
o Laparoscopic Phase: The stomach is mobilized while preserving the right gastroepiploic artery, followed by a celiac trunk lymphadenectomy. This stage concludes with a mini-laparotomy to fashion the tubular gastroplasty(gastric conduit).
 Cervicotomy Phase: Through a left cervical incision, the cervical esophagus is dissected. The surgical specimen and the gastric conduit are then pulled up to perform the cervical anastomosis.
This approach is specifically indicated for squamous cell carcinoma of the upper and middle thoracic esophagus, as well as cases requiring extended lymphadenectomy due to suspected supraclavicular or cervical nodal invol.
