Resumen:

La cirugía de la glándula tiroides ha experimentado una evolución extraordinaria desde sus orígenes históricos hasta convertirse en un procedimiento seguro y altamente especializado. Los primeros intentos quirúrgicos, marcados por una elevada morbimortalidad, estuvieron condicionados por la ausencia de anestesia, antisepsia y técnicas hemostáticas eficaces. La consolidación de la cirugía tiroidea moderna se produjo a partir del siglo XIX gracias a avances fundamentales en la medicina y al trabajo de cirujanos pioneros que establecieron los principios anatómicos y técnicos que aún hoy rigen esta disciplina.
En la actualidad, la cirugía tiroidea constituye el pilar fundamental en el tratamiento del carcinoma de tiroides y de múltiples patologías benignas, integrándose en un abordaje multidisciplinar. La incorporación progresiva de nuevas tecnologías ha permitido reducir complicaciones y mejorar la preservación funcional. Entre las principales innovaciones destacan los dispositivos avanzados de energía para la hemostasia, la monitorización intraoperatoria del nervio laríngeo recurrente, la autofluorescencia de las glándulas paratiroides y las técnicas de ablación percutánea.
Paralelamente, los avances en el diagnóstico molecular y en la comprensión de la biología del cáncer de tiroides han impulsado una medicina de precisión que influye directamente en la planificación quirúrgica, la indicación terapéutica y, en escenarios seleccionados, en el empleo de terapias dirigidas y estrategias neoadyuvantes.
Este artículo revisa la evolución histórica de la cirugía tiroidea, analiza las innovaciones tecnológicas y reflexiona sobre su impacto en la práctica quirúrgica, subrayando que, pese al progreso tecnológico, la experiencia del cirujano y el conocimiento anatómico continúan siendo esenciales para alcanzar resultados óptimos.

Abstract:

Thyroid surgery has undergone an extraordinary evolution from its historical origins to become a safe and highly specialized procedure. Early surgical attempts, characterized by high morbidity and mortality, were limited by the lack of anesthesia, antisepsis, and effective hemostatic techniques. The consolidation of modern thyroid surgery began in the nineteenth century, driven by major advances in medicine and by the work of pioneering surgeons who established the anatomical and technical principles that still govern this discipline today.
At present, thyroid surgery represents the cornerstone of treatment for thyroid carcinoma and for a wide range of benign thyroid disorders and is integrated within a multidisciplinary approach. The progressive incorporation of new technologies has contributed to reducing complications and improving functional preservation. Major innovations include advanced energy-based devices for hemostasis, intraoperative monitoring of the recurrent laryngeal nerve, parathyroid autofluorescence, and percutaneous ablation techniques.
In parallel, advances in molecular diagnostics and in the understanding of thyroid cancer biology have promoted precision medicine, directly influencing surgical planning and therapeutic decision-making and, in selected scenarios, the use of targeted therapies and neoadjuvant strategies.
This article reviews the historical evolution of thyroid surgery, analyzes current technological innovations, and reflects on their impact on contemporary surgical practice, emphasizing that, despite technological progress, surgical experience and thorough anatomical knowledge remain essential to achieve optimal outcomes.
