Coexistence of mediastinal ectopic parathyroid adenoma and thyroid cancer: an unprecedented case

Resumen:
La coexistencia simultánea de un adenoma paratiroideo y un cáncer papilar de tiroides, ambos de localización ectópica mediastínica, representa una entidad excepcional no descrita previamente. Se presenta el caso de una mujer de 70 años con hiperparatiroidismo primario diagnosticado tras estudio por fractura ósea, cuyos análisis mostraron hipercalcemia y elevación de la paratohormona (PTH), la ecografía cervical mostraba un nódulo tiroideo benigno y el PET/CT con 18F-colina una captación focal intensa en mediastino anterosuperior, atribuida a un adenoma paratiroideo.
En una primera intervención, mediante abordaje por cervicotomía de Kocher, fue extirpado un nódulo retroesternal informado como carcinoma papilar de tiroides en la biopsia intraoperatoria, por lo que se realizó hemitiroidectomía izquierda y linfadenectomía central. También se realizó exploración de las paratiroides cervicales con resección de la inferior derecha por tamaño patológico, pero sin conseguir descenso de la PTH intraoperatoria.
Un nuevo PET/CT con 18F-colina confirmó la persistencia de la captación mediastínica previa, por lo que se realizó una segunda intervención con apoyo del servicio de cirugía torácica y asistidos del sistema PTeye. En esta ocasión se resecó un adenoma paratiroideo ectópico en mediastino superior con descenso adecuado de la PTH intraoperatoria. La evolución postoperatoria fue favorable con normalización de la calcemia y de la PT

Abstract:
The simultaneous coexistence of a parathyroid adenoma and a papillary thyroid carcinoma, both located ectopically within the mediastinum, represents an exceptional entity not previously described in the literature. We present the case of a 70-year-old woman with primary hyperparathyroidism diagnosed during the evaluation of a pathological bone fracture. Laboratory tests revealed hypercalcemia and elevated parathyroid hormone (PTH) levels. Cervical ultrasound demonstrated a benign-appearing thyroid nodule, while an 18F-choline PET/CT scan showed intense focal uptake in the anterior superior mediastinum, initially attributed to an ectopic parathyroid adenoma.
The patient underwent an initial surgical procedure through a Kocher cervicotomy. A retrosternal nodule was identified and resected; intraoperative frozen-section analysis reported papillary thyroid carcinoma, prompting completion of a left hemithyroidectomy and central neck lymphadenectomy. During the same operation, exploration of the cervical parathyroid glands was performed. The right inferior parathyroid gland was removed due to pathological enlargement; however, intraoperative PTH levels failed to show an adequate decrease.
Due to persistent hyperparathyroidism, a repeat 18F-choline PET/CT scan was performed, confirming the previously observed mediastinal uptake. A second surgical intervention was undertaken with the collaboration of the thoracic surgery team and the assistance of the PTeye system. On this occasion, an ectopic parathyroid adenoma located in the superior mediastinum was successfully resected, achieving an appropriate intraoperative PTH decrease.
