Resumen:

Los tumores hepáticos primarios en edad pediátrica son infrecuentes y presentan una gran heterogeneidad histológica. El hepatoblastoma constituye la neoplasia hepática maligna más frecuente en niños y suele asociarse a elevación de alfa-fetoproteína (AFP). No obstante, existen entidades tumorales raras que pueden simular esta patología tanto clínica como radiológicamente.
Presentamos el caso de un varón de 14 años diagnosticado inicialmente de hepatoblastoma epitelial variante colangioblástica en su país de origen, con niveles de AFP persistentemente normales. Tras tratamiento quimioterápico inicial fue remitido a nuestro centro, donde se realizó reevaluación radiológica y posterior resección quirúrgica mediante hepatectomía derecha ampliada.
El estudio anatomopatológico definitivo reveló un tumor epitelial-estromal calcificante en nidos (Calcifying Nested Stromal-Epithelial Tumor, CNSET), una neoplasia hepática primaria extremadamente rara considerada por la Organización Mundial de la Salud como tumor de bajo grado y linaje incierto.
Este caso ilustra la importancia del abordaje multidisciplinar en tumores hepáticos pediátricos, el papel de la resección quirúrgica completa como tratamiento fundamental y las dificultades diagnósticas asociadas a entidades tumorales hepáticas poco frecuentes.

Abstract:

Primary liver tumors in the pediatric population are uncommon and encompass a wide spectrum of histological entities. Hepatoblastoma is the most frequent malignant liver tumor in children and is usually associated with elevated alpha-fetoprotein (AFP) levels. However, rare hepatic neoplasms may clinically and radiologically mimic hepatoblastoma.
We report the case of a 14-year-old male initially diagnosed with epithelial hepatoblastoma, cholangioblastic variant, in his country of origin, with persistently normal AFP levels. After initial chemotherapy treatment, the patient was referred to our center for further evaluation. Imaging studies revealed a large hepatic mass involving the right lobe, and the patient underwent extended right hepatectomy.
Definitive pathological examination revealed a calcifying nested stromal-epithelial tumor (CNSET), an extremely rare primary hepatic neoplasm considered by the World Health Organization as a low-grade tumor of uncertain lineage.
This case highlights the importance of multidisciplinary management in pediatric liver tumors, the role of complete surgical resection, and the diagnostic challenges posed by rare hepatic neoplasms.
