Resumen:

La invasión del tronco celíaco en tumores del cuerpo pancreático ha sido considerada clásicamente como un criterio de irresecabilidad. La neoadyuvancia y la cirugía vascular han permitido ampliar indicaciones en centros expertos. Caso clínico: Se presenta un varón de 68 años con adenocarcinoma ductal del cuerpo pancreático con invasión del tronco celíaco, tratado con quimioterapia neoadyuvante y posterior pancreatectomía corporocaudal con esplenectomía, resección del tronco celíaco y reconstrucción arterial de la arteria hepática común. La evolución fue favorable. Discusión: La adecuada selección, la respuesta radiológica y la experiencia HPB avanzada fueron clave. Conclusión: El procedimiento constituye una alternativa válida en pacientes seleccionados.

Abstract:

Celiac axis involvement in pancreatic body tumors has traditionally been considered a criterion for unresectability. However, neoadjuvant therapy and vascular surgical techniques have expanded resection options in selected patients. We report a 68-year-old man with locally advanced pancreatic body ductal adenocarcinoma involving the celiac axis. After neoadjuvant gemcitabine plus nab-paclitaxel and partial radiological response, he underwent distal pancreatectomy with splenectomy, en bloc celiac axis resection, and common hepatic artery reconstruction. Postoperative recovery was uneventful, with discharge on postoperative day 7. Careful patient selection, favorable treatment response, and advanced HPB expertise were key to success. The modified Appleby procedure with arterial reconstruction is a feasible and oncologically sound option in selected patients.
