Abstract:

Introduction: The treatment of metastatic rectal cancer has evolved in recent years toward a multidisciplinary approach in which the combination of systemic therapy and surgical resection of the primary tumor and metastases can improve survival.
Objective: To present the clinical case of a patient with metastatic rectal cancer treated using a multimodal strategy, along with a review of the most recent European guidelines.
Clinical case: A 49-year-old woman diagnosed with stage IV rectal adenocarcinoma with liver metastases. Following a staging workup and evaluation by a multidisciplinary gastrointestinal tumor committee, she underwent systemic chemotherapy, antiangiogenic therapy, surgery for the primary tumor, and subsequent resection of distant metastases.
Results: The patient responded well to neoadjuvant therapy, allowing for R0 resection of the primary tumor and the liver metastases via laparoscopic low anterior resection and segmentectomy of segment 7. During follow-up, she developed pulmonary and ovarian metastases, which were treated with a right upper lobectomy and right oophorectomy—including the tumor mass—and systemic therapy. Currently, after 41 months of follow-up, she has shown pulmonary progression and continues to receive systemic therapy with fluorouracil, leucovorin, and bevacizumab, while under close monitoring.
Conclusion: The management of metastatic rectal cancer must be individualized through a multimodal approach based on the integration of systemic therapy with surgery for the primary tumor and metastases. Sequential resection of metastases may help to control the disease and, in selected patients, may be associated with prolonged survival.
